Early Post-operative CT-Angiography Imaging After EC-IC Bypass Surgery in Moyamoya Patients

Objective: To evaluate the clinical value of early post-operative computed tomographic angiography (CTA) after direct extracranial-intracranial (EC-IC) bypass surgery in moyamoya patients. Methods: A retrospective analysis all adult patients treated at our center from 2013 to 2019 with a EC-IC was performed. Early CTA (within 24 h surgery) compared conventional digital subtraction (DSA) 6–12 months surgery. If available, magnetic resonance time-of-flight (MR-TOF) evaluated 3 and post-operatively as well. Imaging results were analyzed CTA, MR-TOF DSA, whereat DSA used final definite modality decide on patency. Results: total 103 bypasses 63 analyzed. All inclusion criteria met 32 (53 bypasses). In 84.9% appeared definitively, 5.7% uncertainly 9.4% not patent according CTA. suggested definitive patency 86.8% 93.5% showed 98.1% bypasses. The positive predictive (to correctly detect an occluded bypass) 12.5%, negative 100% sensitivity specificity 86.5%. Conclusion: has high confirm bypass. On other hand, false rate (according CTA) can be seen. This must considered critically when initiating possible therapeutic measures.
